Complete your dependent household form

You’ve been selected to complete federal verification. Verification is the process of confirming the information reported on your Free Application for Federal Student Aid (FAFSA). As part of that process, we will verify information regarding your household size. To provide this information, complete the form on this page.

Once you’ve completed the form, please submit it to the Office of Financial Aid and Scholarships at the address listed on the form. Please allow up to 15 business days for your documentation to be reviewed. Additional documentation may be requested, and any form that is incomplete or provides discrepant information will require follow-up and delay processing. If we are unable to complete verification, we may be required to cancel any existing financial aid awards you might have received.

Instructions for filling out the form

If your legal parents are not married to each other:

• Answer the questions based on the parent who will provide more than 50 percent of your support from July 1, 2023 through June 30, 2024.
• If the parent providing the support has married or remarried, stepparent information is required.
